Consultancy NHS Trusts Improving patient safety Imperial College Health Partners - coproducing patient safety in forensic and intensive care settings West London Collaborative were commissioned by Imperial College Health Parters to deliver coproduction on the following wards at West London Mental Health NHS Trust - Solaris and Barron Low Secure Wards - Ealing.Askew Psychiatric Intensive Care Unit - Hammersmith & Fulham The first stage of the work was delivered by researcher Josephine Ocloo and service user consultant Nash Momori. Stage two was delivered on the wards by Nash and Patrick Nyikavaranda. The aim of the work was to coproduce a patient safety measurement and coproduce an intervention. A report would then be compiled. The project was managed by Jane McGrath and Phase 3 was delivery of a Coproduction Activity Report which was delivered to West London Mental Health Trust and Yinka Makinde at Imperial College Health Partners. The final report was called; Patient Safety Measurement and Monitoring Project: West London Mental Health Trust Phase 3 – Co Production Activity Report 2016 and was authored by Patrick Nyikavaranda & Jane McGrath
